NO SAME DAY REFUNDS

ln consideration for being allowed to enter and use the play urea, inflatables, soft play equipment and/or participation in any party or program at TERRE HAUTE BOWLING CENTER, INC. (STRlKERZ), the undersigned on his or her behalf, and on the participant(s) identified below, acknowledges, understands, and agrees to the following conditions:

1. I represent that I am the parent of legal guardian of the participant(s), named below, or I have obtained permission from the parent or legal guardian of the participant(s) named below to execute this waiver on their behalf. I agree that the participant(s) named below shall comply with all of TERRE HAUTE BOWLING CENTER & STRIKERZ terms. In addition, if I observe any hazard during our participation, I will bring it to the attention of the STRIKERZ staff immediately. 

2. I am aware that there are inherent risks associated with participation at STRIKERZ play area, inflatables, soft play equipment and/or participating in the parties and programs. I, on behalf of myself, and the participant(s) named below, knowingly, voluntarily, and freely assume all such risks, both known and unknown, including those that may arise out of the negligence of the participants. 

3. I, for myself, and the participant(s) named below, and our respective heirs, assigns, administrators, personal representatives and next of kin hereby release and hold harmless, TERRE HAUTE BOWLING CENTER, INC & STRIKERZ, there affiliates, officers, members, agents, employees, other participants and sponsoring agencies from and against any all claims, injuries, liabilities, or damages arising out of or related to our participation at STRlKERZ in the use of play area, inflatables, soft play equipment and/or participating in the parties and programs. 

4. By providing email address, I consent to receive promotional emails regarding events, offers, receipts and news from The Terre Haute Bowling Center
